Randall Park Mall Closing

As you may have heard, the Randall Park Sears and our practice there is closing. Our last day for appointments will be Friday, May 1st. But even though our practice is closing, your care remains our concern. Beginning May 4th, we will continue providing personal care for you and your family at the following location:

To ensure continuity of care, please download and fill out the attached Patient Record Release Form so that we can forward copies of your dental records to the practice of your choice. You will need to complete a separate form for each member of your family who is or has been a patient at our Randall Park office. If you do not send us a form, your records will automatically be transferred to our corporate office. If you would like your records forwarded to another dental care provider, you can use the same form.
